Background:

  • A 30-year-old previously healthy male presented with palpitations, nausea, sweating, and presyncope.
  • Symptoms were linked to episodes of nonsustained ventricular tachycardia.
  • No history of smoking, excessive alcohol use, illicit drug use, or familial cardiac disease.

Observation:

  • Diagnostic investigations included electrocardiography, laboratory tests, electrophysiological studies, echocardiography, coronary angiography, chest radiography, cardiac MRI with late gadolinium enhancement, chest CT, lymph-node biopsy, Ziehl Nielsen staining, blood and sputum cultures, and Heaf testing.

Findings:

  • The patient was diagnosed with tubercular myocarditis.
  • Management involved antitubercular chemotherapy, antiarrhythmic and steroid pharmacotherapy, and cardioverter-defibrillator implantation.
  • Repeated imaging was used to monitor disease progression.

Implications:

  • This case highlights the importance of considering rare infections like tuberculosis in the differential diagnosis of ventricular tachycardia.
  • Early diagnosis and appropriate management of tubercular myocarditis are crucial for patient outcomes.
