Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy

Sigmoidoscopy and laparoscopy are distinct medical procedures that enable physicians to internally inspect different parts of the GI tract. Although they serve different purposes, each is essential for diagnosing and, in some cases, treating various medical conditions.
Sigmoidoscopy
Sigmoidoscopy is a diagnostic procedure that uses a flexible sigmoidoscope equipped with a light source and camera to examine the rectum and sigmoid colon. The procedure involves inserting the tube through the anus...

Laparoscopic cholecystectomy is safe for patients 80 and older, despite a higher conversion rate and longer hospital stay. This gallbladder surgery offers good outcomes for the elderly population.

Area of Science:

  • General Surgery
  • Geriatric Surgery

Background:

  • Octogenarians represent a growing demographic requiring surgical intervention.
  • Laparoscopic cholecystectomy is a viable surgical option for elderly patients.

Purpose of the Study:

  • To evaluate the outcomes of laparoscopic cholecystectomy in patients aged 80 years and over.
  • To compare surgical outcomes in octogenarians versus younger patients.

Main Methods:

  • Retrospective review of 1263 laparoscopic cholecystectomies, with 21 patients aged 80+.
  • Data collected included age, gender, comorbidities, American Society of Anesthesiologists (ASA) score, conversion rates, complications, and hospital stay.

Main Results:

  • The >/=80 group had a higher ASA score (52% vs 32%, P=0.047) and a significantly higher conversion rate (19%) and longer hospital stay (4.9 days).

Related Experiment Videos

  • Morbidity was higher in the elderly group (14% vs 3%, P=0.03), including bleeding, wound infection, and atelectasis. Mortality was zero in both groups.
  • Conclusions:

    • Laparoscopic cholecystectomy is safe and well-tolerated in octogenarians.
    • Elderly patients undergoing this procedure experience increased morbidity and longer hospital stays but have zero mortality.
