Background:

Implementing evidence-based programs is crucial for health promotion professionals.
Translating complex, environmental strategies into practice presents significant challenges.
Documenting community change processes is essential for program evaluation.

Purpose of the Study:

To provide health promotion practitioners with a method to evaluate community change.
To document successful applications of environmental strategies in community settings.

Main Methods:

The community change chronicle method involves five steps: logic model development, outcome selection, programmatic data review, material collection and analysis, and story dissemination.
Validation involved a youth empowerment model and development of eight community change chronicles.
Data were collected from 2001 to 2003.

Main Results:

Eight community change chronicles were developed, documenting successful interventions.
Documented successes include tobacco-free school policies (n=2), voluntary secondhand smoke reduction policies in youth venues (n=3), and broader policy/program changes in diverse communities (n=3).

Conclusions:

The community change chronicle method is a validated approach for evaluating environmental strategies in health promotion.
This method facilitates the documentation and dissemination of successful community-led health initiatives.
It aids in bridging the gap between evidence-based research and practical application in community change.
