  • Chronic renal dysfunction (CRD) affects 11.7% of liver transplant recipients.

Purpose of the Study:

  • To identify risk factors for chronic renal dysfunction (CRD) after liver transplantation.
  • To evaluate the impact of CRD on patient survival.
  • To assess the role of immunosuppressive agents in CRD development.

Main Methods:

  • Retrospective analysis of 1173 liver transplantations.
  • Definition of CRD: serum creatinine increase ≥ 1.8 mg/dL for ≥ 2 weeks.
  • Multivariate logistic regression to identify independent risk factors.

Main Results:

  • Early-onset CRD (3-12 months) significantly decreased 5-/10-yr survival rates (66%/46%).
  • Late-onset CRD (>12 months) did not impact survival (98%/86%).
  • Cyclosporine A (CyA) was the sole independent risk factor for CRD; alcoholic cirrhosis and prior renal dysfunction were also associated.

Conclusions:

  • Early CRD post-liver transplant is a critical factor negatively affecting survival.
  • Cyclosporine A-based immunosuppression poses a higher risk for CRD compared to tacrolimus (FK).
  • Mycophenolate mofetil (MMF) rescue therapy showed limited benefit in established CRD, highlighting the need for early detection strategies.
