Background:

  • Mesenteric desmoids are rare, locally aggressive fibroblastic tumors.
  • Colectomy is a surgical procedure to remove part or all of the colon.
  • Familial polyposis is an inherited condition predisposing individuals to colon cancer.

Observation:

  • Two cases of mesenteric desmoids occurring post-colectomy are presented.
  • One case involved a patient with known familial polyposis.
  • The other case lacked a confirmed familial history.

Findings:

  • Aggressive fibrous proliferation characteristic of desmoid tumors was observed.
  • The extensive fibrous changes in one case precluded the creation of a Kock reservoir, a type of ileal pouch anal anastomosis.

Implications:

  • Mesenteric desmoids represent a significant, albeit rare, postsurgical complication after colectomy.
  • The potential for desmoid development should be considered in patients with familial polyposis undergoing colectomy.
  • Surgical planning must account for the possibility of desmoid tumors, which may alter reconstructive options.
