Sleep-disordered breathing (SDB) is prevalent in lung transplant candidates. While lung transplantation can improve oxygenation, new cases of SDB may emerge post-surgery.

Purpose of the Study:
- Determine SDB prevalence before lung transplantation.
- Evaluate the impact of lung transplantation on SDB.
Main Methods:
- Polysomnography (PSG) performed on 117 lung transplant candidates.
- SDB defined by respiratory disturbance index (RDI) or nocturnal oxygen desaturation.
- Repeat PSG post-transplant in 25 subjects.
Main Results:
- 40% of patients on room air (n=80) had SDB.
- SDB resolved in 6/11 patients post-transplant.
- 4/14 patients developed new-onset SDB post-transplant.
- Pre-transplant SDB did not affect survival to or after transplant.
Conclusions:
- Sleep-disordered breathing is common in lung transplant candidates.
- Lung transplantation can improve oxygenation but may lead to new SDB onset.
