Background:

Accurate prediction of intraocular lens (IOL) power is crucial for successful cataract surgery and achieving desired refractive outcomes.
Current IOL prediction models rely on various parameters, but their accuracy limits and sources of residual error require ongoing investigation.

Purpose of the Study:

To quantify the current accuracy limits of intraocular lens (IOL) prediction.
To analyze the residual errors in IOL power calculations.
To propose future strategies for improving IOL prediction accuracy.

Main Methods:

Employed numerical ray tracing using manufacturer-provided IOL data.
Utilized a simple scaling model for predicting postoperative IOL position, validated against other approaches.
Compared prediction errors across multiple IOL models and patient collectives using optical biometry for axial length measurements.

Main Results:

Mean prediction errors were +0.13 D and -0.13 D, with mean absolute errors of 0.44 D and 0.50 D.
Errors showed no systematic bias for short or long eyes, but varied with the IOL position model.
Prediction error differences among IOL types were below manufacturing tolerances and human recognition thresholds.

Conclusions:

Advanced physical models of the pseudophakic eye reduce the need for parameter adjustments.
Future accuracy gains can be achieved through individual corneal topography, pupil size measurements, and improved crystalline lens position prediction.
Tighter manufacturing tolerances for IOLs will also contribute to enhanced prediction accuracy.
