Summary

Revision ankle fusion using ring external fixation offers successful limb salvage for complex cases. This method provides a viable alternative when internal fixation is not feasible, leading to good patient outcomes.

Ankle Joint

The ankle is formed by the talocrural joint (crural = leg). It consists of the articulations between the talus bone of the foot and the distal ends of the tibia and fibula of the leg. The superior aspect of the talus bone is square-shaped and has three areas of articulation. The top of the talus articulates with the inferior tibia. This is the portion of the ankle joint that carries the body weight between the leg and foot. The sides of the talus are firmly held in position by the articulations...

Area of Science:

  • Orthopaedic Surgery
  • Limb Reconstruction
  • Arthrodesis Techniques

Background:

  • Current ankle fusion recommendations prioritize internal fixation (screws/plates).
  • Revision ankle arthrodesis after failed primary fusion presents challenges.
  • Ring external fixation is explored as an alternative salvage technique.

Purpose of the Study:

  • To evaluate the efficacy of ring external fixation in revision ankle arthrodesis.
  • To compare outcomes of different revision strategies for failed ankle fusions.
  • To assess limb salvage rates and functional improvements.

Main Methods:

  • Retrospective review of 45 patients undergoing revision tibiotalar arthrodesis.
  • Treatment groups included repeat internal fixation, ring external fixation, and tibiotalocalcaneal arthrodesis.

Treatment of Ankle Osteoarthritis with Total Ankle Replacement Through a Lateral Transfibular Approach

  • Radiographic union rates and American Orthopaedic Foot and Ankle Society (AOFAS) scores were assessed.
  • Main Results:

    • Overall union rate was 88.9% across all methods.
    • Ring external fixation achieved an 84.6% union rate in revision and re-revision cases.
    • Average AOFAS ankle-hindfoot score improved significantly from 31.1 to 65.8.

    Conclusions:

    • Revision tibiotalar arthrodesis is effective for limb salvage in most patients.
    • Ring external fixation is a valuable option for complex revision ankle fusions.
    • This technique is particularly useful when internal fixation is contraindicated or limited.
