Summary

Patients with multiple sclerosis (MS) can develop chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). Intravenous immunoglobulin therapy improved both central and peripheral nervous system symptoms in these patients, suggesting shared antigenic targets.

Background:

  • Peripheral demyelinating neuropathy is rarely associated with central nervous system demyelinating disorders like multiple sclerosis (MS).
  • Understanding the temporal relationship and shared mechanisms between these conditions is crucial.

Observation:

  • Five patients with definite, relapsing multiple sclerosis (MS) were studied.
  • Deep tendon reflex latency measurements and cerebrospinal fluid (CSF) analysis were performed.
  • Nerve imaging revealed cranial nerve and spinal root enhancement in some patients post-CIDP diagnosis.

Findings:

  • All MS patients developed chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) within 4-22 years.
  • Prolonged deep tendon reflex latencies and elevated CSF protein were observed in all patients at CIDP diagnosis.
  • Intravenous immunoglobulin (IVIG) therapy led to clinical improvement in all patients.

Implications:

  • Combined central and peripheral nervous system involvement in MS-associated CIDP appears responsive to IVIG.
  • These findings suggest potential common antigenic targets in the central and peripheral nervous systems in this patient subset.
  • Further research into shared immunopathogenic mechanisms is warranted.
