Summary

High-resolution 3T MRI is valuable for detecting cerebral lesions in drug-resistant epilepsy. However, adding surface coil imaging did not significantly improve lesion detection or characterization in this patient group.

Background:

  • Drug-resistant epilepsy often requires detailed neuroimaging for surgical evaluation.
  • Previous MRI at lower field strengths (1.0-1.5T) may not detect or fully characterize subtle cerebral lesions.

Purpose of the Study:

  • To evaluate the added benefit of surface coil imaging to high-resolution 3T MRI for detecting and characterizing cerebral lesions in patients with drug-resistant epilepsy.

Main Methods:

  • Twenty-five patients with drug-resistant epilepsy underwent high-resolution 3T MRI.
  • Surface coils were applied over suspected epileptogenic zones after standard head coil imaging.
  • Coil placement was guided by clinical data, video-EEG, and advanced imaging techniques.

Main Results:

  • 3T MRI detected possible epileptogenic lesions in 12 out of 25 patients.
  • Surface coil imaging provided slightly better lesion demarcation but did not reveal new lesions or alter lesion type identification.
  • 3T MRI offered new or additional information compared to previous 1.0-1.5T MRI in 20% of patients.

Conclusions:

  • High-resolution 3T MRI is beneficial for detecting cerebral lesions, particularly malformations of cortical development (MCD), in drug-resistant epilepsy.
  • The additional value of supplementary surface coil imaging at 3T MRI for this indication is questionable.
