    Background:

    • Chylomicrons are key triglyceride carriers.

    Purpose of the Study:

    • To develop and validate a method for measuring chylomicron fractional clearance rates.
    • To investigate chylomicron clearance in individuals with and without hypertriglyceridemia.

    Main Methods:

    • Constant duodenal infusion of emulsified fat (200 mg/kg/hr) for 10 hours.
    • Measurement of triglyceride concentrations in the chylomicron fraction at steady-state.
    • Calculation of fractional removal rates using known input and steady-state plasma levels.

    Main Results:

    • Chylomicron clearance was rapid in healthy individuals (t1/2 = 4.5 ± 2.9 min).
    • Clearance was significantly prolonged in patients with endogenous hypertriglyceridemia (t1/2 = 23 ± 5.5 min).
    • Caloric restriction normalized chylomicron clearance in most hypertriglyceridemic patients, suggesting specific clearance defects rather than generalized TG removal issues.

    Conclusions:

    • The developed method accurately assesses chylomicron fractional clearance rates.
    • Delayed chylomicron clearance is a characteristic feature of endogenous hypertriglyceridemia.
    • Endogenous triglycerides are cleared less efficiently than chylomicron triglycerides, with significant inter-patient variability.
