Insights
Soluble E-selectin levels are elevated in children with active Crohn's disease, indicating its potential as a marker for inflammation activity. This contrasts with soluble L-selectin, which showed no significant differences in active disease states.
Area of Science:
- Pediatric Gastroenterology
- Immunology
- Biomarker Discovery
Context:
- Inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), is characterized by chronic inflammation.
- Leukocyte infiltration in IBD is mediated by adhesion molecules on leukocytes, platelets, and endothelial cells.
Purpose:
- To investigate the clinical utility of serum soluble E-selectin (sE-selectin) and soluble L-selectin (sL-selectin) concentrations in pediatric patients with CD and UC.
- To assess the correlation between these soluble adhesion molecules and disease activity indices.
Summary:
- Serum sE-selectin levels were significantly higher in pediatric IBD patients compared to healthy controls.
- Elevated sE-selectin levels were specifically observed in children with active Crohn's disease compared to those in remission.
- No statistically significant differences in sL-selectin levels were found between IBD patients and controls, or between active and remission states for either CD or UC.
Impact:
- Serum sE-selectin determination shows promise as a valuable tool for estimating inflammation activity in pediatric Crohn's disease.
- This finding could lead to improved monitoring and management strategies for pediatric IBD.
- Further research may explore sE-selectin's role in other inflammatory conditions.
Unlabelled:
Chronic inflammation is the common feature of inflammatory bowel disease. Infiltration of leucocytes is mediated by definite adhesion molecules expressed on the surface of activated leucocytes, platelets and endothelial cells.
Aims:
to investigate the clinical value of determining concentrations of sE-selectin and sL-selectin in children and youth with Crohn's disease and ulcerative colitis.
Patients And Methods:
soluble adhesion molecules were studied in the serum of 31 patients with Crohn's disease (CD) and 27 with ulcerative colitis (UC) and 20 healthy controls aged 6-18. Crohn's disease and ulcerative colitis patients were divided according to Hyams and Truelove-Witts' indeces of disease activity. Serum levels of sE-selectin and sL-selectin were measured using an enzyme-linked immunosorbent assay kit of Bender MedSystem.
Results:
sE-selectin levels of the inflammatory bowel disease patients were higher then those of controls and sL-selectin levels of the IBD patients were lower than those of controls but without statistical significant differences. The sE-selectin levels of Crohn's disease patients with active disease were statistically significant higher then those with remission of the disease (29,17 + 25,65 ng/ml vs 50,74 + 20,00 ng/ml). There was no statistically significant difference in the levels of L-selectin between patients with active disease and those with remission. There were no statistically significant differences in the levels of sE-selectin and sL-selectin between patients with active ulcerative colitis disease and patients with remission.
Conclusion:
determination of sE-selectin in children with Crohn's disease is of significance in estimation of inflammation activity.

Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
