  • Primary intraocular lymphoma (PIOL) is a rare but significant cause of such syndromes.

Observation:

  • Three patients presented with atypical symptoms mimicking viral retinitis, retinal vein occlusion, suspected metastasis, and chronic uveo-retinitis.
  • Initial treatments for presumed conditions were ineffective in these patients.
  • All three patients were ultimately diagnosed with primary intraocular lymphoma.

Findings:

  • PIOL can present with varied clinical signs, challenging differential diagnoses.
  • Diagnostic difficulties arise from its ability to mimic common ocular inflammatory and vascular diseases.
  • Expert tissue handling and analysis are critical for confirming PIOL.

Implications:

  • Increased awareness of PIOL's masquerading potential is needed among clinicians.
  • Prompt and accurate diagnosis through specialized pathology can lead to earlier treatment initiation.
  • Effective management strategies for PIOL require a multidisciplinary approach.
