Tissue plasminogen activator as a hemodialysis catheter locking solution

Tissue plasminogen activator (t-PA) as a hemodialysis catheter lock prevents thrombosis without increasing bleeding risk. This t-PA lock solution maintains catheter function, with only a temporary, safe increase in fibrinolysis.

Purpose of the Study:

- Fibrinogen, D-dimer, and FDPs remained elevated but unchanged post-t-PA administration.
- Euglobulin clot lysis time (ECLT) significantly decreased at 15 minutes post-locking (p=0.016) but normalized by 30 minutes.
- No bleeding episodes or catheter thrombosis occurred; catheter performance was maintained.
Conclusions:
- A 2 mg t-PA locking solution effectively preserves hemodialysis catheter function.
- The observed transient increase in systemic fibrinolysis (decreased ECLT) is temporary and not associated with bleeding risk.
- t-PA represents a safe and effective alternative to heparin for dialysis catheter locking in select patients.
