Summary
Common variable immunodeficiency syndrome (CVID) can present with bronchiolitis obliterans organizing pneumonia (BOOP). Corticosteroid treatment improved lung function and resolved pneumonia in this unique CVID case.
Area of Science:
- Pulmonology
- Immunology
- Internal Medicine
Background:
- Common Variable Immunodeficiency Syndrome (CVID) is a primary immunodeficiency characterized by impaired antibody production.
- Recurrent respiratory infections, particularly pneumonia, are a common manifestation of CVID.
- Bronchiolitis Obliterans Organizing Pneumonia (BOOP) is an inflammatory lung disease with various potential causes.
Observation:
- A 26-year-old female patient with a diagnosis of CVID presented with frequent pneumonia episodes.
- An open lung biopsy was performed, revealing histological evidence of BOOP.
- The patient received corticosteroid therapy for the lung condition.
Findings:
- Following corticosteroid treatment, the patient experienced a complete resolution of pneumonia episodes.
- Radiographic imaging demonstrated significant improvement in lung abnormalities.
- Gas exchange function also showed marked improvement post-treatment.
Implications:
- This case represents the first documented instance of BOOP occurring in association with CVID.
- The findings suggest a potential immunologic basis for BOOP in the context of CVID.
- Further research into the immunopathogenesis of BOOP in immunodeficient states is warranted.
