Background:

  • The Multiple Sleep Latency Test (MSLT) is the gold standard for diagnosing excessive daytime sleepiness.
  • Limited data exists on the reliability of MSLT scoring and diagnostic interpretation.
  • Establishing reliability is crucial for consistent clinical application.

Purpose of the Study:

  • To assess the interrater and intrarater reliability of MSLT scoring.
  • To evaluate the reliability of MSLT in diagnosing narcolepsy and related disorders.
  • To validate MSLT as an objective diagnostic tool.

Main Methods:

  • 44 subjects underwent polysomnography and MSLT.
  • Three independent scorers blindly scored 219 naps.
  • 100 naps were re-scored after six months to assess intrarater reliability.

Main Results:

  • Interrater reliability for sleep latency and REM periods ranged from 0.668 to 0.964.
  • Interrater reliability for narcolepsy diagnosis was 0.883.
  • Intrarater reliability for sleep latency and REM periods ranged from 0.625 to 0.991.

Conclusions:

  • MSLT demonstrates excellent inter- and intrarater reliability in scoring sleep and REM onset.
  • High agreement was found for diagnosing narcolepsy, narcolepsy spectrum disorder, and normal cases.
  • MSLT is a reliable objective measurement tool for various sleep disorders and controls.
