Background:

Medulloblastoma is a rare brain tumor in children.
Cerebellopontine angle (CPA) tumors are uncommon, especially in pediatric populations.
Invasion into adjacent structures like the petrous bone presents significant surgical challenges.

Observation:

A five-year-old Malay girl presented with an extremely rare right CPA medulloblastoma.
The tumor had eroded into the petrous bone and extended into the temporal fossa.
This extensive invasion posed a complex diagnostic and therapeutic problem.

Findings:

A combined subtemporal and retromastoid surgical approach was utilized.
Gross total surgical resection of the medulloblastoma was achieved.
The patient subsequently received adjuvant radiotherapy and chemotherapy.

Implications:

This case highlights the possibility of extensive CPA medulloblastoma invasion in pediatric patients.
The successful application of a combined surgical approach demonstrates its efficacy in achieving complete tumor removal.
Multimodal treatment including surgery, radiotherapy, and chemotherapy is crucial for managing such rare and aggressive pediatric brain tumors.
