Cervical ectopic pregnancy, a rare complication, can be effectively treated with uterine artery embolization (UAE) and immediate dilation and evacuation. This fertility-sparing approach minimizes blood loss and preserves reproductive potential.

Area of Science:

  • Reproductive Medicine
  • Gynecologic Surgery
  • Vascular Interventional Radiology

Background:

  • Cervical ectopic pregnancy is a rare obstetric complication, representing less than 1% of all ectopic gestations.
  • Optimal fertility-sparing management for cervical ectopic pregnancy remains uncertain due to limited case studies.

Observation:

  • A case of cervical ectopic pregnancy in a 32-year-old woman following in vitro fertilization and embryo transfer is presented.
  • The patient had significant risk factors, including Asherman's syndrome and prior uterine surgeries.
  • Conservative management was pursued due to the patient's desire for future fertility.

Findings:

  • A combined approach of uterine artery embolization (UAE) followed by immediate dilation and evacuation successfully terminated the viable cervical ectopic pregnancy.

  • The procedure resulted in minimal blood loss.
  • Fertility capacity was preserved post-treatment.
  • Implications:

    • This case highlights the efficacy of UAE combined with immediate evacuation as a fertility-preserving treatment for cervical ectopic pregnancy.
    • This strategy offers a viable option for women desiring future childbearing after this rare complication.
    • Further research into conservative management of cervical ectopic pregnancy is warranted.
