Summary

A new flash-linking method stiffens corneas similarly to standard ultraviolet A (UVA) cross-linking, but in just 30 seconds. This rapid corneal cross-linking shows comparable efficacy for increasing corneal stiffness.

Area of Science:

  • Ophthalmology
  • Biomedical Engineering
  • Materials Science

Background:

  • Corneal cross-linking (CXL) is a standard procedure to increase corneal stiffness.
  • Standard CXL involves prolonged ultraviolet A (UVA) exposure and riboflavin application.
  • There is a need for faster CXL methods without compromising efficacy.

Purpose of the Study:

  • To compare the stiffness of porcine corneas after standard riboflavin-UVA cross-linking versus a novel, rapid flash-linking method.
  • To evaluate the efficacy of flash-linking in stiffening the cornea using surface wave elastometry.

Main Methods:

  • Porcine corneas underwent either standard riboflavin-UVA CXL (30 minutes) or flash-linking (30 seconds) with a customized photoactive agent.
  • Corneal stiffness was measured using surface wave elastometry (Sonic Eye system) by assessing ultrasound surface wave propagation.

  • Intraocular pressure was continuously monitored during the procedures.
  • Main Results:

    • Both standard and flash-linking methods increased corneal surface wave velocity, indicating increased stiffness.
    • Mean surface wave velocity increased from 90.87 m/s to 109.2 m/s (P=.003) after riboflavin-UVA CXL.
    • Mean surface wave velocity increased from 83.66 m/s to 109.2 m/s (P=.0001) after flash-linking.
    • The observed differences in stiffness increase between the two methods were not statistically significant (P=.74).

    Conclusions:

    • Flash-linking, a rapid corneal cross-linking method, demonstrates comparable efficacy to standard riboflavin-UVA CXL in stiffening porcine corneas.
    • This novel method requires significantly less UVA exposure time (30 seconds).
    • Flash-linking offers a promising, faster alternative for corneal stiffening procedures.
