Summary

Giant aneurysms (GAs) over 25 mm are rare. Fusiform GAs, a rare subtype, have a better prognosis than saccular GAs, especially when presenting with mass effect.

Background:

  • Fusiform GAs commonly affect the internal carotid artery (ICA), basilar, or vertebral arteries and rarely present with rupture.

  • Fusiform GAs, particularly those causing mass effect, demonstrate a more favorable prognosis compared to saccular GAs.
  • Saccular GAs are associated with an 80% mortality rate within a few years of diagnosis.
