Background:

  • Paragangliomas (PGs) are rare neuroendocrine tumors.
  • Accurate cytologic diagnosis and behavior prediction are crucial for patient management.

Purpose of the Study:

  • To detail the cytologic characteristics of paragangliomas (PGs).
  • To identify cytologic features that can predict the behavior of PGs.

Main Methods:

  • Retrospective analysis of 12 cases with cytologic suggestion of PG.
  • Review of smears for cellularity, pattern, cell shape, cytoplasm, nuclear features, and background.
  • Correlation of cytologic findings with clinicoradiologic, histologic, and immunocytochemical data.

Main Results:

  • Cellular smears showed round to oval, plasmacytoid, and spindled cells in various patterns.
  • Pink granules were a consistent feature; pleomorphism was common.
  • Increased mitoses were observed in malignant cases; pleomorphism was less prominent in metastatic tumors.

Conclusions:

  • Cytologic features of PGs are diverse and not solely diagnostic.
  • Site, histology, and ancillary studies are essential for definitive diagnosis.
  • Granules, mild pleomorphism, frequent mitoses, and necrosis warrant careful evaluation for malignancy potential.
