Summary
Posttraumatic syringomyelia (PTS) can develop months after spinal cord injury (SCI). A syringosubarachnoid shunt relieved pain and resolved the syrinx in a rare case of delayed PTS.

- The syringomyelia was attributed to hindered CSF flow at T6 due to vertebral compression post-SCI.
- Syringosubarachnoid shunt implantation successfully relieved pain and resolved the syrinx.
Implications:
- Delayed PTS requires vigilant postoperative follow-up in SCI patients, especially after insufficient decompression.
- Early diagnosis and intervention, such as CSF shunting, are crucial for managing PTS.
