Children with juvenile idiopathic arthritis (JIA) experience disrupted sleep, even during deep sleep stages. Cyclic alternating pattern (CAP) analysis revealed increased sleep disruption in JIA patients, indicating a need for further investigation into sleep disturbances in chronic childhood arthritis.
Area of Science:
- Pediatric Rheumatology
- Sleep Medicine
- Neuroscience
Background:
- Juvenile idiopathic arthritis (JIA) is a chronic inflammatory condition affecting children.
- Sleep disturbances are common in chronic pain conditions, but specific sleep alterations in pediatric arthritis are not well-characterized.
- Cyclic alternating pattern (CAP) is an EEG marker of sleep instability.
Purpose of the Study:
- To evaluate the expression of cyclic alternating pattern (CAP) during slow wave sleep (SWS) in children with JIA.
- To compare sleep parameters, including CAP, between children with JIA and age- and gender-matched healthy controls.
Main Methods:
- Standard polysomnography with conventional sleep scoring and additional CAP analyses were performed on 12 children with JIA and matched controls.
- Participants underwent one night of habituation in a sleep laboratory.
- Sleep efficiency, sleep stage duration, and CAP rate during non-rapid eye movement sleep were analyzed.
Main Results:
- Sleep efficiency and sleep stage durations were similar between JIA patients and controls.
- JIA patients exhibited nocturnal disrupted sleep with increased short awakenings.
- CAP analyses revealed a significantly higher overall CAP rate during SWS in JIA patients compared to controls (P < 0.01), indicating disrupted sleep even during deep sleep.
Conclusions:
- Pediatric patients with JIA experience significant sleep disruption during SWS, as evidenced by increased CAP rates.
- CAP analysis is a valuable tool for investigating SWS disturbances in children with chronic pain conditions like JIA.
- These findings highlight the impact of chronic arthritis on sleep quality in children and suggest potential therapeutic targets.
