Muscles of the Shoulder

The muscles surrounding the shoulder girdle, including the clavicle and scapula, primarily stabilize the scapula. This stable base allows other muscles to move the humerus effectively. Scapular movements often mirror those of the humerus and extend its range of motion. Muscles that Move the Arm

Nine muscles are involved in arm movements. Two of these, the pectoralis major and latissimus dorsi, originate from the axial skeleton and are called axial muscles. The other seven originate from the scapula and are called the scapular muscles.

Background:

  • Deltoid insufficiency can occur after acromionectomy, leading to tendon retraction, adhesions, stiffness, and pain.
  • This condition results from the deltoid muscle separating from its origin and mechanical fulcrum.

Purpose of the Study:

  • To evaluate the clinical outcomes of deltoid reconstruction or deltoidplasty using autogenous tricortical iliac crest bone graft for deltoid insufficiency post-acromionectomy.

Main Methods:

  • Retrospective review of four patients (three male, one female) who underwent revision deltoidplasty.
  • Mean age was 41 years, with a minimum follow-up of 41 months.
  • Outcomes assessed included pain relief (visual analog scale), American Shoulder and Elbow Surgeons score, cosmesis, and complications.

Main Results:

  • Mean pain score improved from 8 to 1 postoperatively.
  • Mean American Shoulder and Elbow Surgeons score improved from 31 to 68.
  • Two patients showed improved cosmesis; one required revision surgery for abductor weakness.

Conclusions:

  • Deltoidplasty reconstruction is a beneficial treatment for deltoid insufficiency after acromionectomy.
