Parkinson disease patients with pathological gambling exhibit overactive brain networks, particularly in the right hemisphere. This dysfunction may stem from dopamine therapy overstimulating reward systems, suggesting gambling is a behavioral addiction.

Background:

Pathological gambling (PG) can emerge in Parkinson disease (PD) patients during dopamine replacement therapy.
The neural mechanisms underlying this phenomenon remain unclear.

Purpose of the Study:

To investigate differences in resting-state brain perfusion between PD patients with active PG, PD controls, and healthy controls.
To identify neural correlates of pathological gambling in Parkinson disease.

Main Methods:

A case-control study involving 11 PD patients with PG, 40 PD controls, and 29 healthy controls.
Resting-state brain perfusion was assessed using single-photon emission computed tomography (SPECT) with technetium TC 99m ethylcysteinate dimer bicisate.
Statistical Parametric Mapping (SPM) was employed for data analysis.

Main Results:

PD patients with PG demonstrated resting-state hyperperfusion in a right-hemisphere network, including the orbitofrontal cortex, hippocampus, amygdala, insula, and ventral pallidum.
No significant areas of reduced perfusion were observed in PD patients with PG compared to controls.

Conclusions:

PD patients with PG exhibit abnormal resting-state dysfunction within the mesocorticolimbic network.
This dysfunction is potentially linked to dopamine therapy-induced overstimulation of reward pathways.
Findings support the classification of pathological gambling as a behavioral addictive disorder.
