Background:

  • Episodic cluster headache (CH) is a debilitating neurological disorder.
  • The precise neurobiological mechanisms underlying CH attacks remain incompletely understood.
  • Previous research has not definitively identified the central nervous system activation centers during CH.

Purpose of the Study:

  • To investigate the cerebral activation centers in patients experiencing episodic cluster headache (CH).
  • To utilize functional magnetic resonance imaging (f-MRI) to map brain activity during CH attacks.
  • To explore the potential of f-MRI in understanding the pathophysiology of primary headaches.

Main Methods:

  • Four patients with episodic cluster headache (CH) underwent functional magnetic resonance imaging (f-MRI).
  • f-MRI scans were performed during asymptomatic states, active headache attacks, and post-sumatriptan administration.
  • Anatomical imaging utilized 3D-MPRAGE sequences, while f-MRI employed echo-planar imaging; data analysis was conducted using BrainVoyager QX software.

Main Results:

  • Significant hypothalamic activation was observed in all patients, ipsilateral to the side of pain during a headache attack.
  • This study represents the first f-MRI investigation to demonstrate the anatomical location of central nervous system activation in CH patients.
  • f-MRI provided a good balance of spatial and temporal resolution for observing brain activity.

Conclusions:

  • The hypothalamus is a key cerebral activation center during episodic cluster headache attacks.
  • f-MRI is a suitable method for investigating the pathogenetic aspects of primary headaches, including CH.
  • While not currently for clinical use, f-MRI holds significant potential for headache research and evaluating pharmacological treatments.
