Summary

Autologous fat transplantation offers a stable, long-lasting solution for facial atrophy, outperforming temporary fillers. Fat grafting shows best retention in areas of fat loss like the cheeks and jaw.

Area of Science:

  • Plastic Surgery
  • Regenerative Medicine
  • Facial Rejuvenation

Background:

  • Facial atrophy presents a significant aesthetic and functional challenge.
  • Current "off the shelf" fillers offer temporary correction for facial volume loss.
  • Autologous fat transplantation is explored as a more durable alternative.

Purpose of the Study:

  • To evaluate autologous fat transplantation as an ideal technique for correcting facial atrophy.
  • To assess the stability and longevity of fat grafts in different facial areas.
  • To understand the principles of structural augmentation using multilayer blunt fat infiltration.

Main Methods:

  • Structural augmentation through multilayer blunt fat infiltration.
  • Focus on volume expansion and resultant tissue shifting.
  • Application of fat grafting to various facial regions including cheeks, periorbital area, and mandible.

Main Results:

  • Fat retention varies across different facial areas.
  • Optimal fat graft "take" observed in regions primarily affected by fat atrophy, such as the cheeks, periorbital area, and mandible.
  • Unpredictable retention noted when fat grafting addresses bony remodeling or mucosal atrophy, particularly in the perioral area.

Conclusions:

  • Autologous fat transplantation is a stable and potentially long-lasting method for correcting facial atrophy.
  • The technique is most effective in areas with primary fat deficiency.
  • Further research may be needed to optimize outcomes in areas involving bony or mucosal changes.
