Summary

Maternal obesity during pregnancy significantly increases risks for adverse pregnancy and birth outcomes, including hypertension and fetal macrosomia. Careful monitoring and pre-conceptional weight reduction are crucial for obese pregnant women.

Background:

Purpose of the Study:

  • To investigate the association between maternal pre-pregnancy Body Mass Index (BMI) and adverse pregnancy and birth outcomes.
  • To examine the influence of maternal obesity on neonatal somatic classification.

Main Methods:

  • Retrospective cohort study utilizing German perinatal statistics (1998-2000).
  • Inclusion of 499,267 singleton pregnancies.
  • Comparison of obese (BMI ≥ 30) versus normal-weight (BMI 18.50-24.99) pregnant women, with obesity categorized into three levels (BMI 30.00-34.99, 35.00-39.99, ≥ 40.00).

Main Results:

  • Obesity (BMI ≥ 30) was present in 10.3% of pregnancies; severe obesity (BMI ≥ 40) in 0.8%.
  • Hypertension, cephalopelvic disproportion, and fetal macrosomia rates significantly increased with higher maternal BMI categories.
  • Pre-eclampsia, gestational diabetes, and fetal structural anomalies were also more frequent in obese women.

  • Pregnancies in obese women require heightened surveillance and meticulous delivery planning.
  • Pre-conceptional weight management is recommended to mitigate risks.
