Summary

This study presents the first case of total left ureteral reconstruction using a cecal appendix in a 3-month-old child. The appendix successfully bridged the defect, demonstrating a feasible surgical option for complex pediatric ureteral reconstruction.

Background:

  • Appendix interposition is a known technique for partial and total ureteral reconstruction.
  • Previous literature has not documented appendix interposition for total left ureteral reconstruction.

Observation:

  • A 3-month-old infant with ureteral necrosis post-pyeloplasty required total left ureteral reconstruction.
  • A cecal appendix was utilized to connect the native kidney's renal pelvis to the urinary bladder.

Findings:

  • The surgical procedure resulted in an uneventful postoperative recovery.
  • Intravenous pyelogram confirmed successful flow through the appendicular graft.
  • The child remained asymptomatic with normal growth at 6-month follow-up.

Implications:

  • Total ureteral reconstruction with cecal appendix is a viable technique in young children.
  • This case establishes a precedent for using appendix interposition in total left ureteral reconstruction.
  • Long-term surveillance is necessary to monitor for potential late complications.
