Abnormal pulmonary arterial pressure limits exercise capacity in patients with COPD

Pulmonary hypertension in COPD patients significantly lowers exercise capacity and impairs gas exchange. This study found elevated pulmonary artery pressure reduces maximum oxygen uptake and exercise performance in COPD individuals.

Purpose of the Study:

- Forty-two patients with moderate-to-very-severe COPD underwent symptom-limited incremental cardiopulmonary exercise testing.
- Right-heart catheterization was performed at rest to measure mean pulmonary artery pressure (mPAP).
- Abnormal pulmonary artery pressure was defined as mPAP > 20 mmHg at rest.
Main Results:
- 32 patients (76%) had elevated resting mPAP (PH group), while 10 did not (NPH group).
- The PH group exhibited significantly lower maximum oxygen uptake (VO2max) compared to the NPH group (785 vs. 1052 ml/min).
- Impaired gas exchange, indicated by increased dead-space ventilation (Vd/Vt) and higher VE/VCO2 ratio, was observed in the PH group.
Conclusions:
- Abnormal pulmonary artery pressure in COPD patients is associated with impaired gas exchange during exercise.
- Elevated pulmonary artery pressure significantly reduces maximum oxygen uptake and overall exercise capacity in individuals with COPD.
- Pulmonary vascular resistance at rest correlates with exercise limitation parameters in COPD.
