Background:

  • Ewing sarcoma (ES) is a rare bone and soft tissue cancer primarily affecting children and adolescents.
  • Gastric involvement is exceptionally rare, with limited documented cases.

Observation:

  • A 44-year-old woman presented with epigastric pain and weight loss.
  • Imaging revealed a mass near the pancreatic tail, later found attached to the stomach.
  • Tumor exhibited characteristics of an anaplastic small-round-cell tumor with specific immunoexpression markers.

Findings:

  • Histopathology confirmed gastric Ewing sarcoma with strong CD99 immunoexpression.
  • Immunostaining revealed markers including S-100, vimentin, PGP 9.5, neuron-specific enolase, and CD117.
  • The patient underwent successful tumor excision without adjuvant chemotherapy.
