Summary

This report details an exceptionally rare case of primary intratesticular leiomyosarcoma in a 65-year-old man. Diagnosis was confirmed via histopathology after orchiectomy, highlighting the need to differentiate from other testicular tumors.

Area of Science:

  • Oncology
  • Uropathology
  • Surgical Pathology

Background:

  • Primary intratesticular leiomyosarcoma is an exceedingly rare malignant tumor arising from smooth muscle within the testis.
  • Distinguishing it from other testicular neoplasms, such as germ cell tumors and paratesticular sarcomas, is critical for appropriate management.

Observation:

  • A 65-year-old male presented with a painless, enlarged right testis.
  • Surgical exploration and high inguinal orchiectomy were performed for diagnosis and treatment.

Findings:

  • Histopathological examination of the resected testicular mass definitively diagnosed a high-grade leiomyosarcoma.
  • Microscopic features confirmed the smooth muscle origin and aggressive nature of the tumor.

Implications:

  • This case underscores the importance of thorough histopathological evaluation in testicular masses to identify rare entities like leiomyosarcoma.
  • Accurate diagnosis is essential for guiding treatment strategies and prognostic assessment in testicular oncology.
  • Awareness of this rare tumor is crucial for clinicians managing testicular abnormalities to ensure comprehensive differential diagnosis.
