Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy01:26

Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy

Sigmoidoscopy and laparoscopy are distinct medical procedures that enable physicians to internally inspect different parts of the GI tract. Although they serve different purposes, each is essential for diagnosing and, in some cases, treating various medical conditions.
Sigmoidoscopy
Sigmoidoscopy is a diagnostic procedure that uses a flexible sigmoidoscope equipped with a light source and camera to examine the rectum and sigmoid colon. The procedure involves inserting the tube through the anus...
Epistaxis01:30

Epistaxis

Epistaxis, or nosebleeds, occurs when small, swollen blood vessels in the nasal mucous membrane rupture. Typically, the anterior septum is the primary site of occurrence.
Etiology
Possible causes of this condition include high blood pressure, trauma, low humidity, upper respiratory tract infections, allergies, foreign bodies, nasal inhalation of corticosteroids or illicit drugs, excessive use of decongestant nasal sprays, facial or nasal surgery, anatomic malformation, tumors, or systemic...
Suctioning the Nasopharyngeal Airway01:29

Suctioning the Nasopharyngeal Airway

Nasopharyngeal suctioning is a procedure to remove secretions from the upper part of the respiratory tract that the patient cannot clear independently. It helps maintain airway patency and prevents complications such as aspiration pneumonia.

Endotracheal Intubation II: Nursing Management01:17

Endotracheal Intubation II: Nursing Management

Endotracheal intubation is a critical procedure that can be lifesaving for many patients with respiratory distress or failure. The role of nursing in managing endotracheal tubes is pivotal, as it involves pre-intubation preparation, assisting during the procedure, and post-extubation care.
1. Nursing Care of Patients Before Intubation
Before the endotracheal intubation procedure, nurses play an essential role in ensuring the process goes smoothly. The nurses must be familiar with intubation...

Cardiomyopathy VII: Pre and Post Operative Nursing Management

Patients with hypertrophic cardiomyopathy (HCM) and left ventricular outflow tract (LVOT) obstruction who remain symptomatic despite optimal medical therapy may undergo a septal myectomy (Morrow procedure). This procedure involves excising a portion of the hypertrophied septum below the aortic valve using a heart-lung machine to improve blood flow through the LVOT. Effective preoperative and postoperative nursing management ensures successful patient outcomes, minimizes complications, and...

Summary

Postoperative amoxicillin/clavulanate did not improve outcomes for functional endoscopic sinus surgery (FESS) patients with chronic rhinosinusitis. This antibiotic treatment did not reduce bacterial growth three weeks after FESS.

Area of Science:

  • Otolaryngology
  • Surgical Infections
  • Pharmacology

Background:

  • Functional endoscopic sinus surgery (FESS) for chronic rhinosinusitis often involves routine postoperative antibiotic use.
  • The actual clinical efficacy of these postoperative antibiotics remains insufficiently evaluated.

Purpose of the Study:

  • To evaluate the efficacy of amoxicillin/clavulanate in the short-term postoperative care of patients undergoing FESS for chronic rhinosinusitis.
  • To assess the impact of postoperative antibiotics on bacterial growth and clinical outcomes.

Main Methods:

  • A randomized controlled study involving 71 patients with chronic rhinosinusitis undergoing FESS.
  • Patients were divided into two groups: one received amoxicillin/clavulanate for 3 weeks post-FESS, while the control group received no antibiotics.
  • Symptom questionnaires, nasal endoscopy, and bacterial cultures were performed before and 3 weeks after surgery.

Main Results:

  • Symptom scores significantly improved in both antibiotic and no-antibiotic groups post-FESS.
  • No statistically significant differences were observed between the groups in symptom scores, endoscopic findings, bacterial culture rates, or antibiotic sensitivity.
  • Bacterial profiles showed statistically significant differences before and after FESS within each group, irrespective of antibiotic use.

Conclusions:

  • Postoperative amoxicillin/clavulanate does not enhance short-term outcomes for FESS in chronic rhinosinusitis patients.
  • Antibiotic administration did not reduce bacterial growth 3 weeks post-surgery.
  • The long-term effects of postoperative antibiotics were not assessed in this study.
