Summary

Partial intracapsular tonsillectomy (PIT) significantly reduces post-tonsillectomy bleeding compared to bipolar diathermy tonsillectomy (BDT). This suggests PIT is a safer technique for tonsil removal in children.

Background:

  • Tonsillectomy is a common pediatric surgical procedure.
  • Post-tonsillectomy bleeding is a significant complication.

Purpose of the Study:

  • To compare the incidence of post-tonsillectomy bleeding between partial intracapsular tonsillectomy (PIT) and bipolar diathermy tonsillectomy (BDT).

Main Methods:

  • Retrospective chart review of pediatric patients (
  • Data collected included bleeding events, demographics, follow-up duration, surgical indications, and comorbidities.
  • Exclusion criteria: incomplete information or less than 2-week follow-up.

Main Results:

  • A total of 677 patients (333 BDT, 344 PIT) were analyzed after exclusions.
  • Post-tonsillectomy bleeding occurred in 6.8% of BDT patients versus 0.98% of PIT patients (p=0.0001).
  • PIT patients had significantly fewer bleeding complications (OR=7.32).

Conclusions:

  • Partial intracapsular tonsillectomy (PIT) is associated with significantly less post-tonsillectomy bleeding than bipolar diathermy tonsillectomy (BDT).
  • These findings support the use of partial tonsillectomy techniques over total techniques to minimize bleeding risk.
