Summary

Actinomyces meyeri cutaneous actinomycosis, a rare condition, can arise from lung infections. This case highlights a pulmonary infection spreading to the skin, successfully treated with long-term antibiotics.

Background:

Actinomycosis is a bacterial infection typically caused by Actinomyces species.
Cutaneous actinomycosis is rare and often a manifestation of disseminated disease.
Pulmonary actinomycosis can present insidiously, sometimes mimicking other conditions.

Observation:

A 51-year-old male presented with a persistent, indurated erythematous chest plaque after pneumonia treatment.
The plaque evolved to fistulization with serohematic drainage containing characteristic white grains.
Initial investigations including bronchoscopy and bronchoalveolar lavage for pulmonary symptoms were negative.

Findings:

Actinomyces meyeri was identified as the causative agent from bacteriologic samples of the cutaneous lesion.
The patient received a 12-month course of combined antibiotic therapy with ceftriaxone and doxycycline.
Complete resolution of the cutaneous actinomycosis and associated symptoms was achieved.

Implications:

This case underscores the importance of considering Actinomyces species in refractory infections, especially with cutaneous involvement.
Prompt diagnosis and prolonged antibiotic treatment are crucial for successful management of Actinomyces meyeri infections.
Highlights the potential for pulmonary actinomycosis to present with secondary cutaneous manifestations.
