Purpose of the Study:

  • To identify causative factors and the etiologic spectrum of active leg ulcers.
  • To determine the prevalence of circulatory disturbances in leg ulcer patients.

Main Methods:

  • Clinical examination of 382 patients with active leg ulcers.
  • Bidirectional Doppler ultrasonography for arterial and venous circulation assessment.
  • Random selection from a population survey of 827 patients.

Main Results:

  • Venous insufficiency was present in 72% of ulcerated legs (38% deep, 34% superficial).
  • Arterial insufficiency (Ankle/Brachial Index ≤ 0.9) was found in 40% of legs.
  • 54% of ulcers were primarily caused by venous insufficiency; 12% by arterial insufficiency; 6% were ischemic; 10% multifactorial.

Conclusions:

  • Venous insufficiency is the dominant cause of leg ulcers.
  • A significant proportion (40%) of leg ulcers result from potentially surgically curable circulatory problems.
  • Objective assessment of all chronic leg ulcer patients is necessary to identify treatable conditions.
