Development of the Oral Microbiota01:28

Development of the Oral Microbiota

The establishment of the oral microbiome begins before birth, challenging the long-held belief that the fetal oral cavity is sterile. The presence of oral microbes such as Streptococcus and Fusobacterium in amniotic fluid suggests that microbial exposure may occur in utero, potentially through translocation from the maternal oral or gastrointestinal tract. This early colonization primes the neonatal immune system and sets the stage for subsequent microbial succession. Maternal health,...
Fetal Circulation01:14

Fetal Circulation

Fetal circulation is a unique system that facilitates the exchange of gases, nutrients, and waste products between the developing fetus and the mother. This intricate process takes place through a special organ called the placenta.
Two umbilical arteries transport blood from the fetus to the placenta. At the placenta, the blood absorbs oxygen and nutrients while simultaneously eliminating waste products. This oxygen-enriched and nutrient-rich blood then returns to the fetus through one...

SBAR II: Application of SBAR

SBAR is an effective communication tool used by healthcare professionals to communicate patient information accurately. SBAR stands for Situation, Background, Assessment, and Recommendation. For a better understanding, an example is given below.
SBAR Report from a Nurse to a Health Care Provider
S: "Hello, Dr. Smith. This is Jane, RN, from the Med Surg unit. I am calling to tell you about Ms. White in Room 210, who is experiencing increased pain and redness at her incision site. Her recent...

Local Anesthetics: Clinical Application as Epidural Anesthesia

Epidural anesthetics are administered in the fat-filled epidural space, the outermost part of the spinal canal. This technique is commonly employed for pain management and anesthesia during lower abdomen and pelvis surgeries or labor and delivery.
Since epidural anesthetics can be infused through an epidural catheter, all types of drugs, including short-acting ones, can be administered. Chloroprocaine and lidocaine are examples of short and long-duration anesthetics, respectively. Bupivacaine...

Summary

Maternal request caesarean sections (CS) have increased significantly in Denmark. Factors like previous birth complications, maternal age, and region are associated with this trend, highlighting a need for better professional support.

Area of Science:

  • Obstetrics and Gynecology
  • Public Health
  • Reproductive Medicine

Context:

  • Rising rates of caesarean sections (CS) in Denmark, including those performed without medical indication (maternal request CS or MRS).
  • Need to understand demographic and obstetric factors influencing the increasing trend of MRS.

Purpose:

  • To analyze the development of MRS over a five-year period (2002-2006).
  • To investigate associations between MRS and parity, prior birth complications, infant weight, maternal age, and geographic region.

Summary:

  • A retrospective analysis of 5,165 MRS cases from 2002-2006 revealed significant increases in MRS for both nulliparous and multiparous women.
  • MRS rates were highest in women aged 35+ (nulliparous) and 15-24 (multiparous), and most frequent in the Capital Region.

  • Previous CS, severe perineal tears, or perinatal death increased the risk of MRS in subsequent deliveries; infant weight showed no influence.
  • Impact:

    • Maternal request CS now constitutes 1.3% of births for nulliparous and 3.6% for multiparous women.
    • Identifies key demographic (age, region) and obstetric history factors associated with MRS.
    • Emphasizes the need for further research into the underlying reasons for MRS to improve professional support for expectant mothers.
