Dysplastic nevi, precursors to melanoma, are more common on vulvar skin than previously believed. Early detection during postpartum checkups is recommended for these pigmented lesions.

Observation:

  • 18 pigmented vulvar lesions were excised from approximately 500 women.
  • Three of these lesions exhibited characteristics of dysplastic nevi.

Findings:

  • The three patients with vulvar dysplastic nevi were young adults (21, 27, 30).
  • All had multiple nevi on torso and limbs; two showed features of dysplastic nevus syndrome.
  • Lesions were larger than 5 mm, brown or black, and often unnoticed by patients.

Implications:

  • Dysplastic vulvar nevi may be underdiagnosed.
