Area of Science:

  • Orthopedics
  • Rheumatology
  • Pathology

Background:

  • Pigmented villonodular synovitis (PVS) is a rare joint disorder.
  • Diagnosis of PVS is often delayed, impacting patient outcomes.

Purpose of the Study:

  • To investigate the clinical course, diagnosis, and treatment of pigmented villonodular synovitis (PVS).
  • To evaluate the effectiveness of early intervention for PVS.

Main Methods:

  • Retrospective analysis of 22 patients diagnosed with PVS between 1973 and 1987.
  • Clinical and radiological follow-up of 19 patients.
  • Review of diagnostic methods, treatment strategies, and patient outcomes.

Main Results:

  • PVS commonly affects the knee, with a higher prevalence in women.
  • Delayed diagnosis was frequent; repeated knee aspirations with dark fluid suggested PVS.
  • Early treatment provided symptom relief, but did not improve long-term prognosis.
  • Aggressive disease courses and arthroses were observed, particularly in early-treated patients.
  • Local resection and synovectomy were primary treatments; irradiation and brisement were used for recurrences and complications.

Conclusions:

  • Early diagnosis and treatment of PVS are valuable for symptom management.
  • Treatment strategies should be tailored to disease localization, symptoms, and severity.
  • Further research is needed to optimize PVS management and improve long-term outcomes.
