Background:

Complex anal fistulas present a significant management challenge.
The Cook Surgisis AFP anal fistula plug was introduced as a potential treatment option.

Purpose of the Study:

To evaluate the efficacy of the Cook Surgisis AFP anal fistula plug in managing complex anal fistulas.

Main Methods:

Retrospective review of patients treated with the anal fistula plug between July 2005 and July 2006.
Data collected included patient demographics, fistula etiology, and success rates.
Success defined as complete closure, absence of drainage, and no abscess formation.

Main Results:

36 procedures were analyzed, with an overall success rate of 13.9% (5 out of 36).

Success rates were 12.5% for cryptoglandular fistulas and 25% for Crohn's disease-associated fistulas.

17 patients (47.2%) required further procedures due to treatment failure, including infection and plug dislodgement.

Conclusions:

The observed success rate of the Surgisis AFP anal fistula plug is significantly lower than previously reported.
Further investigation is warranted to elucidate the reasons for the observed treatment outcomes.
