Summary

Unusual subepithelial corneal infiltrates were observed in Acanthamoeba keratitis patients. These late-onset infiltrates may indicate an immunologic response, mimicking viral or chlamydial infections.

Background:

  • Acanthamoeba keratitis (AK) is a serious microbial infection of the cornea.
  • Corneal infiltrates are common in AK, typically associated with active inflammation.

Observation:

  • Six patients with AK presented with atypical subepithelial corneal infiltrates.
  • These infiltrates appeared late in the disease course, often distant from the primary infection site.
  • Four patients showed infiltrates without other clinical signs of inflammation.

Findings:

  • Subepithelial infiltrates resolved with topical corticosteroids in three patients.
  • Recurrence was noted in two patients upon corticosteroid withdrawal.
  • The infiltrates' presentation resembled those seen in viral and chlamydial keratitis.

  • The findings suggest a potential immunologic mechanism underlying these late-onset infiltrates in AK.
  • This presentation may be mistaken for other infectious keratitis types.
  • Further research into the immunopathogenesis of AK is warranted.
