This case report details a novel non-anatomical intestinal transplantation for a patient with intestinal failure. The successful outcome demonstrates the viability of this technique for complex cases.

Observation:

  • A 38-year-old male with short-bowel syndrome underwent intestinal transplantation.
  • The patient had a unique vascular anatomy (left-sided vena cava).
  • The intestinal implant was positioned in a non-anatomical orientation with 180-degree mesenteric rotation.

Findings:

  • The superior mesenteric artery and vein of the implant were connected to the recipient's aorta and vena cava.
  • Postoperative recovery was excellent, with discharge after six weeks.
  • The patient achieved a significant 10-kg weight gain.

Implications:

  • This non-anatomical intestinal implantation technique offers potential technical advantages.
  • Successful intestinal function in this non-anatomical position validates the approach.
  • This case presents a unique model for future intestinal implant strategies.
