Summary

The Multnomah Community Ability Scale (MCAS) showed poor psychometric fit for measuring community functioning in individuals with severe mental illness. Findings suggest reconsidering its use as a scale and exploring its items for disability assessment.

Background:

  • Community mental health services require outcome measures tailored to community functioning, distinct from medical models.
  • The Multnomah Community Ability Scale (MCAS) was developed to assess broad dimensions of community functioning for clinical and evaluation purposes.

Purpose of the Study:

  • To assess the structural consistency and fit of the MCAS as a measure of community functioning.
  • To evaluate the scale's psychometric properties and invariance over time in a severe and persistent mental illness population.

Main Methods:

  • Confirmatory factor analysis (CFA) was used on a longitudinal sample (n=408) of individuals receiving community mental health services.
  • Exploratory factor analysis (EFA) was conducted, and multiple factor solutions (four-, three-, two-, and one-factor) were tested after item exclusion.

Main Results:

  • None of the previously hypothesized factor solutions achieved a good fit.
  • A high degree of invariance over time was observed, but overall model fit remained poor.
  • Excluding two items did not improve the fit of alternative factor solutions.

Conclusions:

  • The MCAS did not demonstrate adequate psychometric fit as a scale for measuring community functioning in this population.
  • Alternative interpretations of the MCAS are suggested, including using its items as an index for disability assessment.
