A possible relationship between testosterone and lower urinary tract symptoms in men

Lower testosterone levels are linked to more severe lower urinary tract symptoms in men. This suggests testosterone deficiency may play a role in the development of these symptoms and metabolic syndrome.

Purpose of the Study:
- To investigate the association between serum testosterone levels and the severity of lower urinary tract symptoms (LUTS) in men.

- Serum levels of total testosterone, sex hormone-binding globulin, and calculated free and bioavailable testosterone were measured in 278 men.
- Prostate volume and LUTS severity (using International Prostate Symptom Score) were assessed.
- Adjustments were made for age, prostate volume, C-reactive protein, and insulin resistance.

- Calculated free testosterone and bioavailable testosterone showed a negative correlation with overall LUTS and voiding symptoms.
- Lower testosterone levels were significantly associated with the presence of severe LUTS.
- High-sensitivity C-reactive protein and insulin resistance were negatively correlated with total and free/bioavailable testosterone, but not directly with LUTS severity.
Conclusions:
- Endogenous testosterone plays a favorable role in lower urinary tract function.
- Testosterone deficiency may be a contributing factor to LUTS and metabolic syndrome in men.
