Summary

Selenium levels are similar in patients with chronic pancreatitis in India and England. This study found no link between selenium deficiency and pancreatic calculi or diabetes in tropical chronic pancreatitis.

Background:

  • Chronic pancreatitis presents differently in tropical versus temperate regions, with higher prevalence, pancreatic calculi, and diabetes in the tropics.
  • The role of selenium deficiency in these geographical variations of chronic pancreatitis is not well understood.

Purpose of the Study:

  • To investigate if selenium deficiency contributes to the unique features of chronic pancreatitis observed in tropical areas.
  • To compare selenium levels in patients from Madras, India (tropical) and Manchester, England (temperate).

Main Methods:

  • Serum selenium levels were measured using graphite furnace atomic absorption spectrometry.
  • Samples were analyzed from healthy volunteers, chronic pancreatitis patients (calcific and diabetic), and primary diabetes patients from both Madras and Manchester.

    • Selenium bioavailability was found to be equally high in both geographic areas.
    • A significant decrease in serum selenium was observed in patients from both Manchester and Madras (p < 0.001).
    • The similar reduction in serum selenium levels did not correlate with the higher incidence of pancreatic calculi or exocrine failure in the Madras group.

    Conclusions:

    • Selenium deficiency does not appear to be a causative factor for pancreatic calculi or pancreatic exocrine failure in tropical chronic pancreatitis.
    • Observed selenium levels do not explain the accelerated progression to diabetes in tropical chronic pancreatitis patients.
