Summary

A significant percentage of multiple sclerosis patients develop binding antibodies (BAB) and neutralizing antibodies (NAB) against interferon-beta (IFN-beta) treatments. Real-time PCR is recommended for routine NAB measurement in these patients.

Background:

  • Interferon-beta (IFN-beta) is a common treatment for relapsing-remitting multiple sclerosis (RRMS).

  • Monitoring antibody levels is crucial for managing MS patients on IFN-beta therapy.

Purpose of the Study:

  • To determine the frequency of binding antibodies (BAB) and neutralizing antibodies (NAB) against IFN-beta in RRMS patients.
  • To compare different methods for detecting NAB against IFN-beta.
  • To evaluate the clinical utility of real-time PCR for NAB detection.

Main Methods:

  • Sera from 827 RRMS patients treated with IFN-beta were analyzed for BAB and NAB.
  • Binding antibody (BAB) levels were measured.
  • Neutralizing antibody (NAB) detection was performed using MxA protein induction assay (ELISA) and MxA mRNA induction assay (real-time PCR).

Main Results:

  • 43.9% of patients developed BAB after 12 months of IFN-beta treatment.
  • NAB was detected in 70.8% of BAB-positive sera (31.1% of all patients) using real-time PCR.
  • Real-time PCR and ELISA showed comparable results for NAB detection, indicating good correlation.

Conclusions:

  • A substantial proportion of RRMS patients develop antibodies against IFN-beta.
  • Real-time PCR is a reliable and advantageous method for routine NAB measurement in MS patients on IFN-beta.
  • Routine monitoring of NAB using real-time PCR can aid in optimizing IFN-beta therapy for MS patients.
