Ruptured bronchial artery aneurysm associated with bronchiectasis: a case report

A rare case of massive hemothorax caused by a ruptured bronchial artery aneurysm (BAA) was successfully treated. This minimally invasive approach, transcatheter artery embolization (TAE), offers a new treatment option for this condition.

Background:
- Bronchial artery aneurysms (BAAs) are rare, with few reported cases of mediastinal rupture.
- Massive hemothorax typically results from trauma or other known causes.
Observation:
- A 77-year-old female with bronchiectasis presented with anemia, hypertension, hemothorax, and a mediastinal mass.
- Imaging revealed a ruptured 3-cm mediastinal BAA supplying the left lower lobe, causing 2 liters of hemothorax.
Findings:
- Transcatheter artery embolization (TAE) using microcoils was successfully performed to treat the ruptured BAA.
- The patient's condition stabilized after embolization and transfusion, with an uneventful recovery.
Implications:
- Ruptured BAA should be considered in cases of unexplained hemothorax and mediastinal hemorrhage.
- TAE is an effective, minimally invasive treatment for ruptured BAA, offering a viable alternative to surgery.
