Summary

Calciphylaxis, a rare condition causing painful skin ulcers, can affect the penis in patients with end-stage renal disease. This case highlights the importance of considering calciphylaxis in dialysis patients with penile lesions.

Background:

Calciphylaxis is a severe vasculopathy characterized by vascular calcification and medial calcification.
It predominantly affects patients with end-stage renal disease (ESRD) and is associated with significant morbidity and mortality.
Penile involvement in calciphylaxis is uncommon but has been reported.

Observation:

A 64-year-old male patient undergoing peritoneal dialysis presented with painful, necrotic ulcers on the glans penis.
The ulcers developed over a six-month period.
Initial suspicion was atherosclerotic necrosis, leading to partial penile resection.

Findings:

Histological examination confirmed calciphylaxis.
The condition involved calcification and intimal fibrosis in small blood vessels of the penis.
