Insulin: Dosing Regimen and Adverse Effects

Insulin-replacement therapy usually includes both long-acting insulin (basal) and short-acting insulin (to cater to postprandial needs). In a diverse group of type 1 diabetes patients, the average daily insulin dose is typically 0.5-0.7 units/kg body weight. However, obese patients and pubertal adolescents may need more due to insulin resistance.

Summary

Two courses of sublingual immunotherapy (SLIT) significantly increased serum leptin levels in males with allergic rhinitis. This finding suggests SLIT may alter leptin, a hormone previously linked to allergic rhinitis.

Background:

  • Elevated serum leptin is observed in females with allergic rhinitis.
  • A single course of sublingual immunotherapy (SLIT) has not shown an effect on serum leptin levels.

Purpose of the Study:

  • To investigate serum leptin level changes in patients with pollen-induced allergic rhinitis.
  • To assess these changes before and after two pre-seasonal SLIT courses.

Main Methods:

  • Included 41 allergic rhinitis patients and 34 healthy controls.
  • Measured serum leptin levels before and after the second SLIT course.
  • Allergic rhinitis patients were responders to SLIT.

Main Results:

  • Serum leptin levels significantly increased in males after two SLIT courses (p=0.0056).
  • No significant changes were observed in females.

Conclusions:

  • At least two pre-seasonal SLIT courses are needed to significantly alter serum leptin levels.
  • This alteration was observed exclusively in males.
  • Further research is needed to understand the potential protective role of leptin.
