Fulminant endogenous endophthalmitis caused by methicillin-resistant Staphylococcus aureus (MRSA) is rare but serious. Early diagnosis and targeted MRSA treatment are crucial for preserving vision.

- Two patients had MRSA colonization, while one developed bacteremia secondary to phlebitis at an infusion site.
Findings:
- Despite appropriate treatment, two patients experienced irreversible vision loss, with one requiring enucleation.
- One patient achieved functional visual recovery.
Implications:
- Endogenous endophthalmitis due to MRSA, though uncommon, poses a significant threat to vision.
- Prompt and accurate identification of MRSA is critical for initiating effective therapy and potentially preventing vision loss.
